Additional Children (3 mos-17yrs) for urgent care membership pass with parent membership. Visits can be shared with your child's pediatrician or primary care provider to ensure optimal care and follow up. Sports physicals & sick visits are included in membership. Well child visits and well child vaccines are not offered at this time.  

Childrens urgent care pass $25/Month, or may use parents visits and then all visits in excess of those included in the membership would be $25 each for parent and child. 

Yearly Membership includes 40 visits a year and yearly Physical

Monthly Membership is for a minimum of 3 months and  includes a  $99 initial  processing fee. Includes 3 visits a month ( in person or telehealth )

Yearly Membership: includes yearly physical and 40 visits a year. 5% discount and processing fee waived.

We will be offering a Concierge Medicine membership soon  for a limited selection of insurance plans. These Concierge memberships will include a monthly membership fee for the concierge service of $59.00 a month and visits will be charged to your insurance. (DOT physicals, and Aesthetic services will not be included but members will receive a 10% discount.
